Mechanism: Inhib. of CA ® ¯ reabsorption of NaHCO3 in prox. tubule. K+ is exchanged for Na+ in distal tubule. Net = in urinary HCO3-, K+, and water excretion.
Absorption: Oral. Takes effect in 30 min.

Toxicity/S.E.s: Metabolismolic acidosis, hypokalemia. C/i—cirrhosis.
Utility: Treat glaucoma and CNS pressure. Alkalinize urine. Prevent altitude sickness. Diuresis.
Special Features: Relatively weak diuretic. Generally prescribed for non-diuretic purposes. Effectiveness reduced w/continued therapy because plasma [HCO3-] falls.
